Using TennCare's Online Service, Interchange, health care providers can search for TennCare eligibility information for TennCare patients. Long-term health care providers can enter and submit or adjust/void claims, and, upload or download HIPAA transactions.

- Print and sign the completed service agreement.
- Mail the signed agreement to the address on the form.
